Athletes are REQUIRED to complete the following forms for each season.

• CIPPE Form* (PDF)
  (Must complete pages 1-6 at the start of the first sport season. For each subsequent season within the same school year, page 7 needs to be completed. Wrestlers must complete page 9.)

• Athletic Handbook

• NOVA Care Form (PDF)
