Physical Exams & Immunizations for 12th Grade

This has been a difficult time for many and a challenging time for us all. Now more so than ever, it is important for us to make sure our students have met the proper medical obligations for their age group. Whether your child attends the building of Souderton Area High School or the virtual Souderton Area Online Academy, this is a reminder that the Pennsylvania Department of Education requires a signed physical exam completed by your doctor/NP during your student’s 11th grade year. We will accept whatever form your doctor/NP provides for you. Copies of PIAA physicals are acceptable.

Please attach a copy of your student’s immunization records. Students are required to receive the 2nd dose of their Meningitis C vaccine (MCV/Menactra) after the age of 16 years old and prior to the start of 12th grade, as per the PA Department of Health.

The physical, along with immunizations, may be turned into the nurse’s office directly or mailed to the nurse’s office with “Attention: Nurse” on the envelope.

We recommend that you contact your primary care provider to schedule an appointment prior to the start of school. If you do not have a primary care provider or you prefer to have a school physician perform the physical, please notify me in writing. Immunizations are not administered at the time of a school physical.

Thank you for your attention to this matter. If you have any questions, please do not hesitate to reach out to me. If your student had turned in an 11th grade physical with immunizations prior to the school’s shut-down, you do not need to provide another one.
